Novo Nordisk: Obesity momentum must earn back the pipeline premium
Novo Nordisk’s diabetes and obesity business still produces substantial operating profit, but its valuation needs to distinguish established earnings from uncertain pipeline returns. Q2 2026 adjusted sales reached DKK 78.5B, with adjusted operating profit of DKK 33.4B and a 42.5% adjusted operating margin. Yet management’s improved full-year outlook still calls for adjusted sales and operating profit growth between -6% and 0% at constant exchange rates. Meanwhile, pipeline impairments and a cardiovascular trial failure weaken the case for paying generously for future products. As of October 26, 2026, Novo had spent DKK 11.19B on buybacks since February, but the supplied briefs do not disclose cash, debt or free cash flow. The investment case rests on whether obesity demand can sustain today’s profitability while funding the next generation of treatments.
Wegovy carries the clearest evidence of commercial momentum
Novo sells medicines for diabetes, obesity and rare disorders, including insulin, GLP-1 treatments and other biologics. Product sales generate revenue; prescriptions and patient starts indicate demand but do not establish how much revenue or profit the company retains.
The strongest commercial evidence in this window concerns Wegovy. The August results brief reported more than 265,000 weekly US prescriptions for the Wegovy pill for the week ended July 17. Novo also claimed leadership in branded obesity treatments by new patient starts. That supports the case for product adoption, although the materials provide neither competitor prescription figures nor the pricing information needed to test the economic value of that leadership.
The pill launched in the UAE in June and the UK in July. The same brief reported European approvals in July for the pill and Wegovy 7.2 mg. These developments expand the franchise’s formulations and geographic reach.
The limitation is timing. The prescription observation is 101 days old as of October 26. The August 4 financial results are 83 days old. Subsequent briefs update buybacks and denecimig’s regulatory progress, but provide no newer operating results. A same-day repurchase filing does not make July’s demand data current.
The briefs also omit revenue by product, geography and business segment. They support a qualitative conclusion that GLP-1 demand drives the outlook, because management explicitly cited stronger expected GLP-1 sales when raising guidance. They do not establish the precise degree of revenue concentration.
Strong quarterly margins, weaker full-year expectations
The financial picture changes substantially depending on which accounting basis and period the reader selects.

The guidance revision was meaningful. Management moved both adjusted growth ranges from -12% to -4% to -6% to 0%, lifting the midpoint from -8% to -3%. Nevertheless, the revised midpoint still implies contraction. Q2’s positive adjusted growth does not establish a return to sustained annual growth.
Reported figures require particular care. Q2 reported operating profit fell 16% at CER, while adjusted operating profit rose 11%. The brief attributes the divergence to the prior-year US 340B provision reversal and DKK 6.3B of non-cash pipeline-asset impairments.
Those impairments equaled 18.9% of Q2 adjusted operating profit. Monlunabant accounted for DKK 4.0B, or nearly two-thirds of the charge. Excluding these items helps assess current product profitability, but shareholders should still count the loss of expected value from the affected assets.
Management said its non-adjusted full-year guidance midpoints implied sales growth of 5% and operating profit growth of 12% at CER. Those figures should not substitute for the adjusted outlook when estimating recurring growth. The briefs do not provide a complete reconciliation, and the H1 reported margin is not a clean alternative to Q2’s adjusted margin.
The balance-sheet gap also matters. Without cash, borrowings, capital expenditure, working-capital movements or operating cash flow, the materials cannot establish leverage or buyback coverage. Accounting profitability alone does not demonstrate surplus cash.
Pipeline evidence has become less supportive
The most consequential pipeline setback in the supplied window is ZEUS, reported on July 31, 87 days before the as-of date.
In more than 6,300 participants, ziltivekimab reduced markers of IL-6 pathway activity but failed to reduce major adverse cardiovascular events relative to placebo. The hazard ratio was 0.99, with a 95% confidence interval of 0.88–1.11. Serious infections occurred more frequently with treatment, although overall adverse-event rates were similar.
That result supplies no basis for assigning value to a successful ZEUS outcome. Management said it would leave adjusted operating profit guidance unchanged but trigger a Q3 non-cash impairment. The supplied briefs do not quantify that charge.
HERMES and ARTEMIS remain scheduled to continue, with results anticipated in H1 2027. They preserve potential upside, but ZEUS argues against treating that upside as dependable earnings.
Denecimig presents a different risk. On September 17, 39 days before the as-of date, the EMA’s CHMP recommended FREHEMGO for haemophilia A in adults and children, with or without inhibitors. Novo described weekly, every-two-weeks and monthly prophylaxis through a pre-filled pen. It expected initial European launches in Q4 2026 and broader availability in early 2027.
The recommendation is not final European authorization.
The October 26 consolidated brief also reports that the FDA extended its review on October 2 because of manufacturing-facility remediation. According to Novo, the agency had identified no deficiencies in the submitted clinical efficacy or safety data, but provided no new decision timeline. Management’s H1 2027 US launch objective therefore remains conditional.
This distinction matters economically. ZEUS weakens the evidence for a treatment’s expected benefit. Denecimig’s US delay concerns the ability to secure manufacturing clearance and commercialize the product. Neither belongs in the same category as established product earnings.
Buybacks continue, but their funding deserves scrutiny
The October 26 update reports cumulative repurchases of DKK 11.19B for 40.3M B shares since February 4, against an overall authorization of up to DKK 15B over 12 months.
That leaves DKK 3.81B under the overall ceiling. The May 6 programme reports DKK 7.39B spent against its DKK 11.2B limit, also leaving DKK 3.81B. These overlapping programme figures should not be added together.
Cumulative spending increased from DKK 8.08B through August 14 to DKK 11.19B in the latest update, an additional DKK 3.11B. Management has continued returning capital despite the pipeline setbacks.
Gross repurchases do not translate directly into an equivalent reduction in outstanding shares. The August brief reports a separate net transfer of 3.14M shares from Novo through incentive-programme transactions during August 4–7. By October, the company held 54.3M treasury shares, representing 1.2% of share capital.
The capital-allocation judgment remains incomplete. Buybacks can benefit continuing owners when the shares cost less than intrinsic equity value and the company retains sufficient funding for operations and investment. The briefs establish the spending, but not cash coverage or the net-debt position needed to test that judgment.
Valuation: capitalize established earnings, limit pipeline credit
A defensible valuation starts with Q2 adjusted operating profit rather than annualizing the higher H1 reported margin.
Multiplying Q2 by four gives a mechanical annual revenue reference of DKK 314B and operating profit of DKK 133.6B. These are valuation reference points, not forecasts. They ignore seasonality and cannot reproduce full-year guidance because the briefs omit the comparable annual adjusted figures.
Two methods frame the enterprise value:
1. Capitalized normalized free cash flow to the firm: normalized EBIT × assumed after-tax retention × assumed cash conversion, divided by the discount rate less perpetual growth.
2. EV/normalized EBIT: an explicitly selected earnings multiple, without reference to trading multiples or analyst targets.
All valuation amounts below are in DKK. Every tax, conversion, growth, discount-rate and multiple input is an analytical assumption.
| Scenario | Method and explicit assumptions | EV |
|---|---|---:|
| Bear | Cash-flow capitalization: DKK 110B EBIT; 22% tax; 70% conversion of after-tax EBIT into free cash flow; 10% discount rate; 0% perpetual growth | DKK 601B |
| Bear | Earnings multiple: DKK 110B normalized EBIT × 10 | DKK 1,100B |
| Base | Cash-flow capitalization: DKK 133.6B EBIT; 22% tax; 80% cash conversion; 9% discount rate; 2% perpetual growth | DKK 1,191B |
| Base | Earnings multiple: DKK 133.6B normalized EBIT × 12 | DKK 1,603B |
| Bull | Cash-flow capitalization: DKK 145B EBIT; 22% tax; 90% cash conversion; 8.5% discount rate; 3% perpetual growth | DKK 1,851B |
| Bull | Earnings multiple: DKK 145B normalized EBIT × 14 | DKK 2,030B |
The bear case assumes normalized operating profit falls 18% below the Q2 annualized reference. The base case preserves that reference without assigning a separate pipeline premium. The bull case requires operating profit 8.5% above the reference, stronger cash conversion and more durable growth.
The base cash-flow calculation is transparent: DKK 133.6B × 78% × 80% produces DKK 83.4B of assumed normalized annual free cash flow. Capitalizing that at 9% less 2% gives DKK 1,191B.
My central enterprise-value range is DKK 1,190B–1,600B. The spread reflects uncertainty about cash conversion and earnings durability. The wider scenario range shows how costly weak conversion would be, even if accounting margins remain attractive.
These are operating-business values, not equity values. The materials omit net debt and other claims needed to bridge enterprise value to equity value. They also omit an exact outstanding or diluted share count. Treasury shares divided by a rounded ownership percentage would provide only a rough issued-capital estimate, not a reliable per-share denominator. A defensible per-share equity value is therefore unavailable from the supplied materials.
The valuation would be too high if Q2 overstates sustainable earnings, cash investment absorbs more profit than assumed, or competitive pressure reduces realized revenue and margins. It would be too low if Wegovy expansion delivers durable growth with strong cash conversion or the remaining pipeline produces commercially valuable results. The perpetual-growth method also depends on Novo replenishing its product portfolio over time.

Novo’s established business supports substantial enterprise value. The next evidence must show that obesity adoption produces durable cash earnings and that pipeline investment earns an adequate return. Continued buybacks cannot establish either outcome.
